Care Management Console Overview
The Care Management Console brings all elements of a client’s care episode together—referrals, packages, and episode data—in one place. Access it by clicking Care Management at the top of any client’s profile.
1. Unified View
- Quickly see referral details, episode status, and assigned packages.
- Track changes without switching between multiple tabs.
2. Managing Episodes
- Open or update an episode from within the console (e.g., set it to “Active” or add a start date).
- Link referrals and packages to keep everything connected for reporting.
3. Editing Referrals & Packages
- Update referral statuses (e.g., “Accepted”) and assign packages (like “EAP”) in a few clicks.
- Confirm funding or payment details directly in the console.
4. Keeping Data in Sync
- Any changes here automatically appear on the main client profile.
- You can still manage each referral or package individually if needed.
Use the Care Management Console to streamline how you coordinate and monitor client care. This single interface helps you stay organized, save time, and maintain a complete, up-to-date record of each client’s care journey.
