How to create a survey
I will show you how to create forms that you can send to clients, or a practitioner can complete on their behalf. All surveys will be stored on the client profile, under the tab 'survey responses'. And if you set up Custom Data Fields first you have the ability to add important information to the client's Profile Information directly, and use it for your service reporting.
